The HDMI Port is simply there to provide direct output via an HDMI Cable to a Display Screen (LCD Screen or TV).  This can be Video from the Video Capture (Movie) modes, or (on some models) Live-View output, or (again on some models) Still Image Play-back.

The "A/V" Port is there for connection via a USB Cable (USB3 on a few newer models - USB2 on most) to a PC running an Image Capture App (such as BYE/BYN or else EOSUtility or ...).  That PC Application can then both Control the Camera and Download/Display the resulting Images and/Or Video.
